PATIENT HANDBOOK

Mission Statement:
To provide affordable, high quality,
patient-centered health care for 

East Central Oklahoma.

The idea was born in 2003 to apply for federal funding to meet the primary health care of the community.  A group of community leaders came together to dedicate themselves to the effort of applying for the grant.  On the third application, the East Central Oklahoma Family Health Center, Inc. received 330 funding on August 1, 2008.  The center opened its doors for business on January 2nd, 2009 with 10 employees.  Electronic medical records were put into place on October 20th, 2009.  In February 2010,  behavioral health services were initiated.  In July 2010 340b prescription drug coverage began for the patients of ECOFHC.

With the Ground Breaking Ceremony in April 2011, a new chapter was opened for ECOFHC.  This 11, 280 square foot Health Center and a 3,600 square foot Administration Building has allowed ECOFHC to offer more services and provide better health care for the community and patients.  An in-house pharmacy, digital x-ray and certified drug testing has been added to the services available to ECOFHC patients.  On June 2012, ECOFHC was awarded funding for a New Access Point in Henryetta for Okmulgee County.  The site opened on October 1st, 2012.  On July 2015, ECOFHC was awarded funding for a second New Access Point in Wewoka for Seminole County. This site opened on November 16th, 2015
